Everything you need before you arrive.

A little preparation makes your visit smoother. Here’s what to bring, where to find forms, and how to reach us.

What to bring to your visit

Bringing these along helps us give you the most complete picture, and the most accurate recommendation, on day one.

  • A photo ID

    And your insurance card if you have one. Insurance may cover part of your care, and it helps us keep your records straight either way.

  • Your current glasses and contacts

    Plus what you know about your prescription. It tells us where your vision is starting from.

  • A list of medications

    Including anything for your eyes, and any allergies.

  • Your questions

    Write them down beforehand. The consultation is yours, no question is too small.

  • Sunglasses and a driver

    For dilation and especially for surgery days, your eyes will be sensitive and you'll need a ride home.

Paying for your procedure

The premium lens is the part no insurance plan covers, so the lens we recommend is chosen for your eye, not a coverage list. Because that part is private-pay, we can give you the exact price in writing at your consultation, not a range. Where insurance covers part of your care, what you owe depends on your plan’s benefits and deductible, so we’ll confirm that once your benefits are verified. We’re happy to provide documentation for any out-of-network benefits you may have.

Financing with CareCredit

We work with CareCredit, the healthcare financing card, so you can pay over time in monthly payments. Subject to credit approval.
